Nurse Advisor London | United Kingdom (GB) | Full-time | Fix-term
OBJECTIVE
Initiation of Britannia medication when requested. Provide ongoing support for patients on treatment therapies to maximise patient outcomes/benefit. Train patients, HCP’s and carers in treatments and management.

KEY ACCOUNTABILITIES

Initiation of treatments when requested:
  • Ensure Honorary Contract is in place.
  • Ensure receipt of referral documents.
  • Manage referrals for initiation of new patients.
  • Complete a full Pre-Therapy Assessment to ensure the appropriateness of patient (suitability in line with SPC, contraindications).
  • Respond to HCP if patient identified as inappropriate.
  • Follow the process of initiation as per Britannia protocol.
  • Ensure maintenance of referral documentation and associated documentation within the specific Pathfinder data base.


Provide ongoing support for patients on Britannia medication to maximise patient outcomes:
  • Support for patient management until therapy is stopped.
  • Provide skin care management in line with Britannia protocol.
  • Provide stoma care management in line with Britannia protocol.
  • Ensure device and dose adjustment of medication in line with Britannia protocols
  • Liaise appropriately with Accountable HCP to make for any medication review or changes of medication.
  • Ensure updated documentation related to any treatment / management changes is stored in the Pathfinder data base.
  • Coordinate treatment pathway.


Train patients, HCP’s and carers on administering medication and management
  • Give advice and educational support to patients, their carers according to Britannia protocols.
  • Upskill HCP’s on treatment initiation, therapy maintenance and management and optimisation of patient care when requested.
